Make not picking up fields in a form

I’m creating a webhook on Make and trying to pick up the data from a form I’m building. When I’ve run it through tests it only picks up the first question in the form not the second. Any ideas? (I’m new to softr)

Quick update: I’ve also tested this with zapier and I have the same problem
